Wat is hantavirus?

Hantavirussen zijn een familie van RNA-virussen (geslacht Orthohantavirus) die voornamelijk door knaagdieren worden overgedragen. Mensen raken voornamelijk besmet door het inademen van virusdeeltjes uit uitwerpselen, urine of speeksel van knaagdieren. De ziekte verloopt als een van twee klinische syndromen, afhankelijk van de virussoort: hemorragische koorts met nierbetrokkenheid (HFRS) in Eurazië, of hantavirus-pulmonaalsyndroom (HPS) in de Amerika's.

Andesvirus, de stam in deze uitbraak

Gevallen die zijn gekoppeld aan de MV Hondius zijn door het NICD in Zuid-Afrika en het RIVM in Nederland in laboratoria bevestigd als Andes-orthohantavirus (ANDV). Andes is het enige hantavirus met gedocumenteerde overdracht van mens op mens — voor het eerst waargenomen bij uitbraken in zuidelijk Argentinië en Chili. Het veroorzaakt een ernstige vorm van HPS met een sterftekans tot 30 à 50% in gepubliceerde reeksen.

Symptomen en verloop

De incubatietijd bedraagt 1 tot 6 weken (meestal 2 tot 4). Vroege symptomen zijn aspecifiek: koorts, vermoeidheid, hevige spierpijn, hoofdpijn en soms maagdarmklachten. Na 4 tot 10 dagen kan zich snel de cardiopulmonale fase ontwikkelen met hoesten, kortademigheid, vocht in de longen en cardiogene shock — de belangrijkste doodsoorzaak bij HPS.

Behandeling en preventie

Er bestaat geen specifieke antivirale behandeling en geen geregistreerd vaccin tegen het Andesvirus. De zorg is ondersteunend: zuurstof, beademing en hemodynamische ondersteuning op een intensive care. Preventie richt zich op het vermijden van blootstelling aan knaagdieruitwerpselen en, bij deze uitbraak, op contactopsporing onder medepassagiers en bemanning van de MV Hondius.
