Qu'est-ce que l'hantavirus ?

Les hantavirus forment une famille de virus à ARN (genre Orthohantavirus) portés principalement par des rongeurs. Les humains sont infectés essentiellement en inhalant des virus aérosolisés issus de fèces, d'urine ou de salive de rongeurs. La maladie se manifeste par l'un de deux syndromes cliniques selon l'espèce virale : fièvre hémorragique avec syndrome rénal (FHSR) en Eurasie, ou syndrome pulmonaire à hantavirus (SPH) dans les Amériques.

Le virus Andes, la souche de cette épidémie

Les cas liés au MV Hondius ont été confirmés en laboratoire par le NICD (Afrique du Sud) et le RIVM (Pays-Bas) comme orthohantavirus Andes (ANDV). Le virus Andes est le seul hantavirus avec transmission interhumaine documentée, observée pour la première fois lors d'épidémies dans le sud de l'Argentine et du Chili. Il provoque une forme sévère de SPH avec une létalité atteignant 30 à 50 % dans les séries publiées.

Symptômes et évolution

L'incubation dure 1 à 6 semaines (typiquement 2 à 4). Les symptômes initiaux sont non spécifiques : fièvre, fatigue, douleurs musculaires intenses, céphalées, parfois troubles digestifs. Après 4 à 10 jours, la phase cardiopulmonaire peut se développer rapidement avec toux, dyspnée, œdème pulmonaire et choc cardiogénique — principale cause de décès dans le SPH.

Traitement et prévention

Il n'existe ni traitement antiviral spécifique ni vaccin homologué contre le virus Andes. La prise en charge est symptomatique : oxygène, ventilation mécanique et soutien hémodynamique en unité de soins intensifs. La prévention repose sur l'évitement de l'exposition aux excrétions de rongeurs et, dans cette épidémie, sur le traçage des contacts parmi les passagers et l'équipage du MV Hondius.

Remains in quarantine for 30 days from last exposure with retest before release; 45-day total monitoring period.
