What is hantavirus?

Hantaviruses are a family of RNA viruses (genus Orthohantavirus) carried mainly by rodents. Humans are infected chiefly by inhaling aerosolised virus from rodent droppings, urine, or saliva. Disease manifests as one of two clinical syndromes depending on the virus species: hemorrhagic fever with renal syndrome (HFRS) in Eurasia, or hantavirus pulmonary syndrome (HPS) in the Americas.

Andes virus, the strain in this outbreak

Cases linked to MV Hondius have been laboratory-confirmed by NICD South Africa and RIVM Netherlands as Andes orthohantavirus (ANDV). Andes virus is the only hantavirus with documented person-to-person transmission — first observed in outbreaks in southern Argentina and Chile. It causes a severe form of HPS with case fatality reaching 30–50% in published series.

Symptoms and progression

Incubation runs 1–6 weeks (typically 2–4). Early symptoms are non-specific: fever, fatigue, severe muscle aches, headache, sometimes gastrointestinal upset. After 4–10 days the cardiopulmonary phase can develop rapidly with cough, shortness of breath, fluid in the lungs, and cardiogenic shock — the leading cause of death in HPS.

Treatment & prevention

There is no specific antiviral therapy and no licensed vaccine against Andes virus. Care is supportive: oxygen, mechanical ventilation, and haemodynamic support in an intensive care unit. Prevention focuses on avoiding exposure to rodent droppings and, in this outbreak, contact tracing of co-passengers and crew of MV Hondius.
